Dr Darina Oravcová: The Deep Plane Facelift addresses the deep tissue layer located beneath the skin and the muscles of facial expression. This fibro-fatty layer is elevated, mobilized and released from its structural attachments to the bone, allowing the entire complex to be fully repositioned and lifted upward. This specific technique yields a permanent or highly enduring, exceptionally natural-looking rejuvenation. The face retains an inherently natural look following a Deep Plane facelifting procedure.
NewLookHoliday: How does a Deep Plane Facelift differ from more traditional facelift methods?
Dr Darina Oravcová: In a Deep Plane Facelift, this specific fibro-fatty layer is explicitly released from the retaining ligaments that anchor it to the underlying deep facial structures. In contrast, traditional facelifting methods do not manipulate this layer from these deep structural attachments. Instead, they focus primarily on lifting and tightening the surface of this layer itself. While the results of traditional methods may be more short-lived, the procedure remains perfectly appropriate for certain patients.

Dr Darina Oravcová: A Deep Plane facelift is indicated for virtually any degree of facial sagging or tissue ptosis. It allows for a significant anatomical repositioning of the descended tissues back to where they naturally belong.
However, some mature patients prefer to avoid drastic alterations to their appearance due to concerns that they might find it difficult to adapt to a newly rejuvenated face, despite a very beautiful outcome.
In such instances, we can opt for alternative facial lifting modalities that do not involve as extensive a structural shift as the Deep Plane technique, and patients remain highly satisfied with these options. Nonetheless, with a Deep Plane facelift, the face retains an inherently natural look.
The results are exceptionally long-lasting, the incisions heal into highly inconspicuous scars, and the quality of the skin itself appears significantly rejuvenated following a Deep Plane Facelifting.
NewLookHoliday: What are the primary advantages of a Deep Plane Facelift compared to other techniques?
Dr Darina Oravcová: It is well-documented that the results achieved with a Deep Plane facelift are significantly more durable. They undoubtedly endure longer than facelifts performed using standard or less invasive surgical techniques.
Naturally, because the Deep Plane approach manipulates the deeper structural layers of the face, the operative time is longer than other types of facelifts. Patients are fully informed of this during their initial clinical consultation.
NewLookHoliday: What is the expected recovery timeline following a Deep Plane Facelift?
Dr Darina Oravcová: A Deep Plane facelift can be performed under general anaesthesia, local anaesthesia, or intravenous conscious sedation, often referred to as twilight sedation, where the patient does not undergo full general anaesthesia. This involves an intravenous administration that induces a deeply relaxed, comfortable state, allowing the patient to smoothly undergo several hours of surgery without any awareness or distress. The precise duration of the surgery is highly individualized, depending entirely on the comprehensive scope of the procedures performed during the Deep Plane facelift.
Regarding postoperative recovery, patients who undergo the procedure under local anaesthesia or conscious sedation are safely discharged home on the exact same day as their surgery. Their head is wrapped in a bulky compression dressing incorporating soft cotton padding, meaning the entire head is protected by a medical dressing.
Subsequently, the patient returns for a follow-up assessment approximately one week later. If all the sutures are intact and the surgical wounds are healing appropriately, we transition this large head dressing to a specialized chin strap or compression garment. This is an elastic compression hood that provides enhanced comfort and is typically worn for roughly another week. Sutures are then generally removed two weeks post-surgery.
Following this, targeted scar care management is essential. The maturation of scars is an individual process, but it is necessary to plan for dedicated scar maintenance for a minimum of six months to a year until they fully mature and become inconspicuous. However, this routine is not overly restrictive. Camouflage makeup can eventually be applied to the incision lines, rendering the history of the procedure virtually invisible to others.
NewLookHoliday: What are the potential risks or limitations associated with a Deep Plane Facelift?
Dr Darina Oravcová: The clinical risks associated with the Deep Plane facelift technique are broadly comparable to those of other advanced facelift modalities. Although our surgical methods are exceptionally gentle and precise, meticulous care must be taken regarding all anatomical facial structures-most notably the branches of the facial nerve-to prevent intraoperative injury.
It is critical to maintain absolute haemostasis to minimize the risk of significant haematomas or bruising. Nevertheless, despite these precautions, temporary neuropraxia or transient nerve weakness can occasionally occur. This is typically not an absolute severance of the nerve, but rather a temporary impairment due to the close proximity of surgical instruments during deep tissue dissection.
Consequently, some patients may experience a temporary asymmetry in facial expressions postoperatively. This condition is entirely transient and typically resolves completely, returning to baseline within a matter of days or a few weeks.
NewLookHoliday: What aesthetic outcomes can clients expect after a Deep Plane Facelift, and how long do they last?
Dr Darina Oravcová: In the immediate postoperative phase following any facelift or deep plane facial rejuvenation, the patient’s appearance will look unnatural.
This is an expected stage that must be anticipated, as the tissues gradually settle, postoperative oedema (swelling) subsides, and haematomas (bruising) resolve. Initially, the patient will not look camera-ready, but the appearance improves day by day.
Typically, after approximately three weeks, the patient is fully ready to return to social settings without any conspicuous signs that would attract unwanted attention from the public. The long-term results are profoundly enduring.
This category of facelift routinely lasts for years, even decades—easily 10 to 15 years. Providing the patient maintains a healthy lifestyle and adheres to proper skin and scar care protocols, the longevity of the rejuvenation is exceptionally prolonged.
NewLookHoliday: How can you tell if a Deep Plane Facelift has been executed well and naturally, as opposed to creating an artificial appearance?
Dr Darina Oravcová: It is imperative for the surgeon to exercise restraint during tissue elevation to ensure the face does not appear overly taut, windswept, or unnatural, thereby strictly preserving fundamental aesthetic principles.
Ultimately, an expertly performed facelift is one that remains completely undetected, even by immediate family members. I have treated patients whose own families or sisters, with whom they do not interact on a daily basis, completely failed to notice they had undergone a rhytidectomy. They merely observed a subtle, positive change.
They would ask the patient what she had done to her face, where she had been, or if she was utilizing a premium new skincare line. Yet, it never crossed anyone's mind to ask if she had undergone surgery.
This means they noticed absolutely no visible scarring or unnatural distortion of facial features. That is the true hallmark of a superbly executed facelift: its absolute invisibility.
NewLookHoliday: Can a Deep Plane Facelift also address skin quality issues, or should it be combined with other treatments?
Dr Darina Oravcová: Particularly with the Deep Plane technique, the quality of the overlying skin itself shows a notable improvement. This is likely due to the concurrent redraping of the skin alongside the repositioning of the deep fibromuscular system during the lift.
Consequently, the entire face takes on a more youthful vitality. However, if a patient—whether female or male, as men frequently seek this procedure as well—has neglected skin health for years and presents with severe solar damage or advanced structural laxity, a facelift alone will not suffice. While a Deep Plane facelift effectively addresses tissue ptosis (sagging), it does not alter the intrinsic texture or structural quality of the skin surface. The face could look lifted yet retain the same damaged surface texture.
Therefore, it is highly advantageous to combine a Deep Plane facelift, or indeed any facelift, with ancillary skin rejuvenation treatments to ensure the skin surface looks healthy, radiant, and fully restored postoperatively.
NewLookHoliday: Is it possible to combine a DPFL with other plastic surgery procedures?
Dr Darina Oravcová: In most instances, a Deep Plane facelift, or any comprehensive facelift, prompts the consideration of adjunct corrections in other facial aesthetic zones. This is because optimizing one structural area can cause remaining imperfections elsewhere to become more pronounced.
For instance, more noticeable infraorbital hollows (bags under the eyes) may become apparent, or a pronounced elongation of the upper lip or downturned oral commissures may no longer complement the rejuvenated lower face. Therefore, it is highly recommended to combine this surgery with smaller, targeted facial procedures to achieve a harmonious, balanced, and aesthetically consistent outcome.
Standardly, when a Deep Plane facelift is performed, it naturally addresses lateral neck laxity as well; it is exceedingly rare to lift the face in complete isolation. Therefore, a partial neck lift along the sides is inherently integrated into a Deep Plane facelift. Should this prove insufficient—such as in cases with severe cervical laxity, significant submental adipose tissue (double chin), or hypertrophic submandibular salivary glands causing contour deformities along the mandibular angle—an additional surgical approach is required to address the neck comprehensively.
This alternative access is via a discreet submental incision concealed within a natural skin crease beneath the chin, allowing us to perform a specialized Deep Plane Necklift. This procedure directly modifies the platysma, which is the muscle responsible for visible vertical neck bands. Concurrently, subplatysmal fat excision can be performed, and the submandibular glands can be safely addressed or reduced, allowing us to achieve a beautifully defined, sharp cervical contour.
NewLookHoliday: What is the average duration of a Deep Plane Facelift procedure?
Dr Darina Oravcová: Naturally, this is highly variable and depends on whether the procedure is performed in combination with adjunct techniques. Interestingly, as a surgeon gains vast experience with a specific operation, the procedural efficiency increases, reducing the overall operative time. On average, the surgery may require approximately four to five hours when combined with a comprehensive neck lift.
NewLookHoliday: Do you prefer local anaesthesia or general anaesthesia for a Deep Plane Facelift?
Dr Darina Oravcová: Personally, I strongly prefer local anaesthesia combined with intravenous conscious sedation (analgosedation). This approach is significantly gentler on the entire systemic physiology and imposes vastly less cognitive and neurological stress compared to general anaesthesia. Prolonged general anaesthesia inherently introduces greater clinical risks. Deep Plane facelifts are exceptionally well-tolerated and smoothly managed under conscious sedation.
NewLookHoliday: Is an overnight hospital stay required following a Deep Plane Facelift?
Dr Darina Oravcová: When we perform the Deep Plane facelift under conscious sedation, overnight hospitalization is not required. The patient is discharged to return to their accommodation immediately following the procedure. However, it is mandatory that they are accompanied by a companion or chaperone, as they may feel slightly disoriented or unsteady due to the extended period of recumbency and the presence of the compression head dressing.
NewLookHoliday: Do you perform the Deep Plane Facelift on male patients as well?
Dr Darina Oravcová: Yes, absolutely. The procedure can sometimes take slightly longer due to male facial anatomy and hair-bearing skin. While we haven't seen a massive surge or "boom" in male patients seeking this surgery domestically in Slovakia yet, a small percentage of our clientele consists of men, and those who do undergo the procedure are predominantly international patients coming from abroad.
