What does SMAS facelift mean?

SMAS facelift involves the area from the scalp downwards to the ear area, furthermore to earlobes to the occipital hairline. Neck skin, jawline, lateral cheeks, midface and nasolabial wrinkles as well as eyebrow tail can be lifted by SMAS surgery.

This technique is suitable for clients in their late 40s, 50s or older with presenting forehead laxity and face aging, such as facial dropping.

SMAS facelift technique

The procedure itself involves a long incision lead from the area of scalp to the skin by ears continuing around earlobes to the hairline margin.

Preauricular skin and fat layer is detached first so that subcutaneous fascia gets fully exposed and separation of the layer of frontal nasolabial folds to the cheekbone is proceeded. Ear neighboring superficial fascia is dissected and the opened area serves as entering for penetrating gate below superficial fascia and deep myofascial layer.

These steps allow the surgeon to tighten and lift the superficial fascia towards to hairline margin by a complete separation of deep myofascial and superficial fascia. Then the fascia is fixed to the pre and retro auricular fascia. Excessive and dropped skin is gently removed so that the incision is nice and smooth.

Recovery and results after SMAS facelift

This surgery affects deep layers and is more complex than simple facelift. It involves deep plane myofascial separation, thus the results are a lot more efficient. On the other hand it does not affect muscle motion ability either facial expressions.

As it is a more complex surgery and covers relatively large area, the recovery takes longer, swelling may persist too and incision scars take longer to fade. However, a precise aftercare will shorten the recovery period (up to 50%) and benefits the results too.

This technique enables a greater lifting and tightening of fascia than any other facelift technique and it is a long-lasting surgery (up to 10 years).

Summary

The SMAS facelift is performed in general anesthesia and involves a long incision starting at the scalp along the ear area to the retroauricular hairline margin. Recovery period takes seven to 10 days and stitches are removed after ten to fourteen days.

A client is not allowed to drink or eat on the day of surgery and anticoagulants should be suspended one week ahead of the planned surgery.

Smoking and alcohol drinking may have (and often do have) a negative impact on the expected result and are advised to be omitted for about a month after the surgery. Clients after SMAS facelift should avoid extensive neck exercise for three months after the surgery.

Common complication is a temporary nerve injury which is naturally eliminated shortly after the procedure. Among others there is a low possibility of poor incision healing (in 3%-5%) and higher level of swelling due to larger and deeper operative area. These drawbacks disappear in the recovery period in most cases.

Benefits are highly likely to exceed the drawbacks as SMAS facelift has most significant tightening effect and its results last for over a decade.
